Immunization shots record
Legal Guardianship/Power of Attorney (if applicable)
Physical Examination (done within one year)

EXPEDITE: To expedite the Registration/Enrollment process please download, print and fill in the "Student Enrollment Form" found below. It is Mandatory to fill all three pages of the Form.
